The Colan TOS era addresses this as Stark was always wanting to marry, but deeming it unfair to have a wife because of his health situation.  Apparently they were going for a tormented rich guy.

At the moment, the only 2 women I could see Tony marrying are Bethany Cabe and the Black Widow. 

I can't see Tony marrying.  Ever.  And I think Bethany Cabe is a great character and a perfect fit for Tony Stark.  But I also think that something should always come between the two of them staying together.  Because Tony Stark settled down isn't Tony Stark.

Spider-Man is another one I think is better off single.

Well then, which characters can you guys imagine settling down and marrying and having kids?  Hulk was apparently one, judging by the Planet Hulk storyline.

Didn't Stark almost marry Masque at one point.  My memory is poor, but I thought they were pretty heavy until her dad died at Avengers mansion.

I believe the Iron Age origin story had him engaged, with Joanna Nivena leaving him after Tony decided to maintain his Iron Man identity.

Marianne Rogers was also identified as Tony's fiance.
